Our policy regarding our client's confidentiality.

ClientConfidentiality

Privacy of Information: All information shared by clients during counselling sessions will be kept confidential except as required or permitted by law.

Limits to Confidentiality:  Confidentiality may be broken under the following circumstances:

  • If there is a reasonable suspicion of abuse or neglect of a child, elderly person, or dependent adult.
  • If the client poses a threat of serious harm to themselves or others.
  • As required by court  order or where otherwise legally mandated.

Record Keeping: Client records and session notes will be stored securely and will only be accessible to  authorised staff members.

Sharing of Information:  Information may be shared with other healthcare professionals or entities only with the explicit written consent of the client, except where otherwise permitted or required by law.

Client Access to Records:  Clients have the right to access their records, except in instances where such access may cause harm to the client or others.

By engaging in counsellingservices with Optimise Potential, you acknowledge and agree to the terms ofthis confidentiality agreement. You understand the limits of confidentialityand the circumstances under which your information may be shared.
